The Canadian Association for Community Living found that 95% of students with disabilities receive inclusive education in public schools, however only 48% of young adults with an intellectual disability were included in their society (CaCL, 2010). This finding needs to change to create a truly inclusive culture as urged by the UN Convention on the Rights of Persons with disabilities (2006). The webinar discusses the unique role of occupational therapy within a school-based team to successfully transition students with disabilities to post-secondary options¸ including independent living, education, employment, and community participation. The webinar will elaborate on aspects of working collaboratively within interprofessional teams.

After taking part in this webinar, participants will:

1. List challenges related to the transition of students with disabilities to meaningful post-secondary options.

2. Describe strategies that can be used by school-based teams to facilitate the transition of disabilities to meaningful post-secondary options.

3. Share strategies to develop positive and collaborative relationships among the team members.
