Family caregivers play an important role in supporting individuals with a variety of disabilities but it is not standard clinical practice to prepare or support them in their caregiving role. As a result, many experience stress that can compromise their own health and well-being as well as the care they provide. Caregivers support individuals with disability as they transition across care environments and, as a result, their needs for support change. This webinar will introduce a model of care – the Timing it Right Stroke Family Support Program – and discuss adaptation for other illness populations.

After taking part in this webinar, participants will:

1. Understand caregivers’ support needs and how they change across the care continuum.
2. Consider a model of caregiver support and mechanisms for intervention delivery.
3. Consider application of the model to a broad range of illness populations.
